, exercise, active/mental games, music, crafts, etc.
) for elderly seniors.
Schedule: Standard operating hours are 8:00 am – 4:30 pm; Monday-Friday.
Please note work hours may fluctuate depending on the Participant and business needs, flexibility is required.
Hourly Wage: $15.

after 90 Days Duties/Responsibilities: Under the supervision of the Center Manager plans, directs and carries out a program of activities that provide opportunities to enhance optimal levels of physical, cognitive, and social functioning (i.
e.
, exercise, active/mental games, music, crafts, etc.
) Assist Participants in the selection of activities within their restrictions or limitations.
Encourage hobbies; stimulate resident interest and social communications.
Plans and implements group activities, specific recreational programs, religious and social activities.
Posts schedule of monthly activities.
Charts all pertinent Participant information relative to the Activity Department in the Participant’s chart at least quarterly (assessments/progress notes).
Collaborates with the Center Manager to prepare the annual departmental budget re: activities and supplies.
Participates in the Interdisciplinary Team’s initial assessments, care planning, and ongoing re-evaluations of the Participant’s case.
Report any changes regarding Participants.
Participates in the coordination of a 24-hour care delivery.
IDT Team Member.
Participate in and support Quality Improvement initiatives.
Other duties as assigned by the Center Manager or other designee.
Required Skills/Abilities: Must be able to deal tactfully and patiently with personnel, participants, family members, visitors, government agencies/personnel, and the public.
Communicates with the medical staff, nursing personnel, and other department supervisors.
Ability to organize work and follow through on assigned tasks Must possess the ability to make independent decisions when circumstances warrant such action.
Must have the cognitive ability to process and perform tasks for Participant safety.
CPR and First Aid Certification.
Excellent verbal and written communication skills.
Education and Experience: High school diploma or equivalent plus one year of experience in the human services field and skills to work with the frail/elderly population.
One year of experience working with the frail or elderly population, or willing to undergo training to supplement this requirement.
Requirements: Requires valid State driver’s license with a clean driving record.
Successful completion of background check, physical, drug screening, TB Screening Physical Requirements: Works in well-lighted/ventilated office and therapy areas.
Sits, stands, bends, lifts, and moves intermittently during working hours.
Is subject to frequent interruptions.
Must be constantly alert for Participant’s safety.
Attends and participates in continuing educational programs.
Is subject to lifting, carrying, and supporting Participants.
May include overnight travel for in-service training programs, etc.
Must be able to lift, push, pull, and move a minimum of 25 pounds.
Must be able to assist with the evacuation of Participants.

Employment Requirements: Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer’s expense).
Successful completion of post-offer employment drug screening and physical (at employer’s expense).
CPR and First Aid Certification, or willing to obtain at employer’s expense.
Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.
This position has full access to Participant, program, and staff records and reports, requiring discretion to protect individuals and the program.
