* Monitor and report changes in patients' conditions to the interdisciplinary nursing team.
* Assist with the development and implementation of individualized care plans.
* Document patient information accurately and comprehensively.
* Maintain patient confidentiality and adhere to HIPAA regulations.
* Act as liaison and maintain positive relations with residents and their families and physicians.
* Contributes to/modifies the patient care plan.
* Organizes/prioritizes routine and non-routine nursing care activities.
* Adheres to standards of practice and safe technique in preparation and administration of medications.
* Develops and maintains effective relationships through effective and timely communication.
