*Position Summary*: The primary function of this position is two fold: First, is to serve as a resident advocate so as to ensure each residents psycho-social , spiritual needs and wishes are met and maintained from the time of admission through discharge, and, secondly, to market the facility and Its services in such a manner as to attract new referrals and admissions to the facility.
Both these primary functions are provided in accordance all federal, state and local standards, guidelines and regulations which govern the facility, and, as directed by the Administrator so as to ensure accurate rights and needs of each resident is met on a continuum.
*Education and Experience Requirements*: Bachelors degree in Social Work, or, bachelors degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, psychology, etc.
and, one year of supervised social work experience in a health care setting working directly with individuals.
Ability to read, write and speak English.
Ability to follow oral and written direction *Duties and Responsibilities including but not limited to:* *General: 75%* 1.
Interviews prospective clients and presents all information pertinent to admission to the facility.
2.
Conducts facility tours when requested or as necessary.
3.
Makes all necessary arrangements with hospital staff, referring entities, etc.
, to ensure a timely and smooth admission process.
4.
Completes all necessary admission paperwork with each resident and/or responsible party.
5.
Competes all discharge planning making appropriate referrals upon discharge for the residents continuing care, equipment needs, home health, transportation and medications.
6.
Initiates resident room changes as necessary or requested and documents satisfaction or failure.
7.
Promotes public relations in the community.
Attends networking meetings at hospitals, aging services and community events.
8.
Participates on the Interdisciplinary Care Committee and completes the initial assessment MDS and care plan for each resident.
9.
Evaluates all psychosocial needs of the resident and creates a plan of care addressing any such.
10.
Initiate family caregiver group monthly 11.
Records in progress notes, any changes in condition, payor class and/or psychosocial needs.
12.
Reviews antipsychotic medications of residents.
Assume appropriate diagnosis for use.
*Administration: 25%* 13.
Organizes and directs all functions of the Social Service and Admissions Departments.
14.
Codes all accounts payable properly for timely processing.
15.
Maintains department budgets within established parameters.
16.
Completes necessary paperwork and maintenance of records including but not limited to, weekly hours, budget variance reports, time studies, statistivs, performance improvement, etc.
17.
Reviews, revises and establishes new policies and procedures for the department(s).
18.
Adheres to, upholds and enforces all facility policies and procedures.
19.
Adheres to, upholds and enforces all regulations and recommendations of the Department of Health, the Health Care Financing Administration, the Department of Public Welfare.
20.
Keeps apprised of new information in the field by attending seminars/affiliation meetings, reading related literature, etc.
, as directed by the Administrator.
21.
Serves on the Performance Improvement Committee and other committees as assigned.
22.
Presents staff in-services as needed and/or assigned.

*Physical Demands and Working Conditions:* 1.
Must meet the general health requirements set forth by the policies of the facility including a medical and physical examination.
2.
Must possess sight and hearing senses at all times or use prosthetics, which will enable these senses to function adequately so that the requirements of the position can be fully met.
3.
Must be able to walk and sit for extended periods of time.
4.
Other demands include frequent arm and finger movements.
5.
Required to reach, handle and manipulate equipment on occasion.
6.
Moves intermittently throughout the workday.
7.
Works in and around all areas of the facility.
8.
Is subject to frequent interruptions and must then adjust schedule accordingly.
9.
May be subject to handling of and exposure to hazardous chemicals.
10.
May be subject to exposure to infectious waste, blood and body fluids, diseases, conditions, etc.
, such as TB, AIDS, and Hepatitis B viruses.
11.
Will be subject to emotionally distraught residents, family members, visitors, personnel, etc.
and must possess the ability to cope with the stresses of the environment.
12.
Must be able to work extended hours to accommodate the position requirements if necessary and on all three shifts to be available for the remainder of the workforce as necessary.
